Registering with a GP practice
Finlayson Street Surgery are currently informally managing our patient list. We are not accepting registrations directly at the surgery at this time (new born babies can still be registered directly with the same practice as their mother)
HOW TO REGISTER - If you have internet access
- Go to NHS inform - registering with a GP Practice
- Complete the registration form
- Email it to: NSS.PSDTeam1Enquiries@nhs.scot
HOW TO REGISTER - If you don't have internet access
- Collect a paper form from Reception
- Return completed forms to Fraserburgh Hospital
- Hospital will forward form to NHS Practitioner Services
** PLEASE DO NOT BRING COMPLETED FORMS TO SURGERY **
What Happens Next?
- You will receive confirmation from Practitioner Services when your registration is processed
- You will be allocated to either ourselves or Saltoun Surgery
