To help us offer the appropriate advice, please complete the Pre-travel Form below and hand in to Reception.  You will then have to contact the surgery 7 days later for advice regarding your travel arrangements and to arrange any appointments that are necessary.

We recommend that a minimum of 8 weeks prior to your travel is needed for health/vaccination advice and the administration of vaccines (if required). We appreciate that there may be emergency travel situations. Please let us know as soon as possible and we will try our best to accommodate your needs.

This is a private (non NHS) Clinic. The advice for travel is provided free of charge but there may be charges for some travel vaccinations. There is also a charge for Yellow Fever Exemption Medical Certificates.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. Information regarding charges can be found by clicking on the Travel Vaccination Info document below.
